CARRIER DATABASE - REQUIRED DOCUMENTS

• Certificate of Contract Carriage Authority as issued by the Interstate Commerce Commission (ICC).

·        Signed contract  

o       Please fill in date, company’s legal name, address, and MC#.

o       Initial each page, and sign with name and title at the end of the contract. 

• Current Cargo, Worker's Compensation, and Liability insurance certificates as issued by your insurance agent with an additional insured endorsement naming our company, TOP FLIGHT TRANSPORTATION, INC. as certificate holder.   These certificates must be faxed or sent from your insurance company.

 

• Copy of Cargo policy exclusions MUST also be provided.
• If your company is not required by law to carry Worker’s Comp Insurance, please completely fill out and sign the Company Worker's Comp Insurance Statement.

 

• Completed IRS W-9 form (If your company is Incorporated please fill out TIN request enclosed).

 

·       Carrier Information Sheet.

 

·       Insurance Company Information Sheet.

 

·       Carrier Request Cover Letter.
